Australian Immunisation Register

If you have had vaccines at our clinic in the past but some or all of them aren’t showing on your Medicare vaccination record, the most likely reason is that you had them before the Australian Immunisation Register was introduced for the whole of Australia.

If you are an existing patient, and would like to have your vaccines added to the register, please complete the form below. We will endeavour to have this done within two business days, subject to our other workload.

Please note that while we can add Yellow Fever vaccines to the register, this alone will not be considered a valid travel document in countries where yellow fever vaccination is required. If you have lost your yellow book and need a new yellow fever certificate issued, you will need to call the clinic on 07 32219066. Our doctors can replace a lost paper vaccine certificate but there is a $25 re-issue fee.
